Inspector Notes
An unannounced inspection was conducted 12/19/2023 in response to a complaint regarding staffing and supervision and hazardous substances and other harmful agents. Observations, documentation, and interviews were conducted. The preponderance of evidence supported the allegations of noncompliance with standards and law. The complaint is determined to be valid.
Please sign the violation notice and complete the ?plan of correction? and ?date to be corrected? for violation cited on the violation notice and return it to me within 5 business days. The plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

2cleaning material be kept in a locked place preventing access by children.
Evidence:
The laundry closet located in the center?s hallway was open and contained laundry detergent with the warning label to keep out of reach of children.
Evidence:
In a statement dated 11/30/2023, staff member, S1 documented ?I had inadvertently brought my JUUL vape pen into the school, having forgotten to remove it from my dress pocket. Unfortunately, this item fell out of my pocket in the Beginners 1 classroom and was later found by a child.? The vape pen was observed in the hand of child C1, age 2, and was removed from the child by a teacher.
