During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 6/13/2025, as a part of probation. The Department issued a Special Order on 3/12/2025 to place the licensee on probation for a period of six months as a result of high-risk violations.
The on-site inspection began at 10:00 a.m. and ended at 12:20 p.m. The inspector reviewed compliance in the areas listed above. There were 47 children present and 11 staff. The inspector reviewed six children?s records and five staff records on site. This inspection included document review (i.e. daily schedule, menu, license, previous inspection, attendance records, injury reports, emergency drill logs, and medication authorization forms), tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
