Inspector Notes

An unannounced inspection was conducted on 10/25/2023 based the center?s report of an incident that occurred on September 26, 2023. The preponderance of the evidence gathered during the investigation support non-compliance with standards.

A violation notice was issued. Please sign the violation notice and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 11/06/2023. Please specify how the
deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or

Based on review of classroom video provided by the center and staff interviews, it was determined that staff used the forbidden action of rough handling of children.
Evidence:
On September 26, 2023, S1 roughly handled two children. S1 used her hand on a child?s forehead to redirect him. She moved another child by his arm to redirect him to a carpeted area; when the child did not follow her directions, she picked him up under his arms and sat him forcefully on the classroom stairs.
