During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on and completed on 10/29/2024. The on-site inspection began at 11:45 a.m. and ended at 2:43 p.m. The inspector reviewed compliance in the areas listed above. There were 96 children present and 17 staff. The inspector reviewed 9 children?s records and 5 staff records on-site.

This inspection included document review (i.e. Injury logs, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/06/2024. A POC submitted after this date will not appear on the public website.

If a child aged seven months to 18 months has a physical examination prior to attendance, it must be dated within three months prior to the first day attendance. Child #3, aged 13 months, had a physical examination that was dated more than 10 months prior to the first day of attendance.
Plan of Correction: Update parents on the timeline of physicals and have all admin check dates

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #1 and Staff #4 have not completed the training. Staff #1 has been working at the center for more than five months and Staff #4 has been working at the center for more than seven months.

Lockdown procedures shall be practiced at least annually. The center had no documentation of lockdown drills.
