Inspector Notes
Unannounced inspection was conducted this date to investigate a self report that was received on 08/16/2023 regarding care. Staff interviews conducted, observation of the Infant 1 classroom and observation of the center's video recording was made. A violation was cited related to information that was provided in the self report. Other violations were found that was not relative to the self report.

3Standard 8VAC20-780-340-A
Based on staff statements and observation of the center's own video recording system, one staff failed to ensure the care of a child.
Evidence:
1. On 08/14/2023 Child #1 was given another child's infant bottle and consumed approximately 4 ounces of breast milk.
2. The center's bottle feeding policy, the policy requires that staff look at the name on the bottle and verify with another staff that the correct bottle is given to the correct child.
3. Both staff are required to document the verification in a daily report.
4. The video footage on 08/14/2023 and 08/15/2023 showed Staff #1 giving an infant bottle to Child #1 without verifying that the infant bottle was the correct bottle.

Plan of Correction: Retraining of bottle verification procedure will be done for all staff that work in infant room. The management team will do regular checks to ensure process is being followed.

Based on the center's own video recording system, staff failed to wash their hands after diapering.
Evidence:
On 08/15/2023, Staff # 1 and #2 failed did not wash their hands after diapering children.

Based on observation of the center's own video recording system, proper cleaning and sanitizing of a diaper surface was not completed.
Evidence:
On 08/15/2023, staff failed to clean the diaper surface on three occassions with soap and water and sanitizing with bleach and water.
