Inspector Notes

A complaint investigation was initiated on 07/01/2024 and concluded on 07/02/2024. A complaint was received by the department regarding allegations in the areas of staffing and nutrition.

The preponderance of evidence gathered during the investigation did not support the allegations and the complaint was determined not valid.

During the course of the investigation, other non-compliances were determined. The violations cited were not part of the complaint investigation.

Please address a corrective action plan for each violation found and return the singed Inspection Report to me within 5 working days.

Standard 8VAC20-780-350-Q
Based on staff statements, a parent's written permission and a written assessment was not found for one child who was assigned to a different age group.
Evidence:
1. The PreK classroom is designated for children ages 4 to6 years old.
2. Child #1 (3 years) was transitioned to the PreK classroom in April 2024.
3. A parent's written permission, a written assessment by the Director and lead teacher was not found.
Plan of Correction: We will reach out to the former teacher to obtain documents of assessment of a child and have all parties sign.
Standard 8VAC20-780-560-L
Based on observation of the center, staff did not sit with children during meal times.
Evidence:
1. Three staff were present in the classroom.
2. One staff was observed sitting on the floor on the carpeted area. A second staff was observed diapering an infant. The third staff was observed sanitizing a high chair tray.
3. During this time, 3 infants were observed sitting in low high chairs eating.
Plan of Correction: Remind every staff member who is in our infant classroom that they must be seated with infants while they are eating.
Standard 8VAC20-780-60-A
Based on review of records, the center did not maintain a complete record for one child.
Evidence:
A parent's written permission to assign Child #1 to a different age group was not maintained in the child record.
Plan of Correction: We will ensure the record is complete. We will have the parent sign off on the assessment.
Standard 8VAC20-780-80-A
Based on review of the attendance record, the record of daily attendance was not written and did not document the arrival of each child.
Evidence:
1. The arrival of one child in the Intermediate Class was not recorded on the daily attendance record as the arrival occurred.
2. The Center's record of attendance is electronic.
Plan of Correction: We will supply every classroom with forms to report arrival and departure of every child in the classroom.
