Inspector Notes

An unannounced monitoring inspection was initiated and completed on 07/11/2024 in response to a self-report received by the licensing office on 07/10/2024 relating to forbidden actions. The inspection was completed jointly with another local agency. The inspector reviewed 2 children?s records and 1 staff records on-site on 07/11/2024. This inspection included interviews, observation of two classrooms and document review.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-E
Based on staff observations and staff statements, the licensee did not ensure that the center's own policies and procedures were maintained.
Evidence:
1. The center is required a policy for reporting suspected child abuse as required by ? 63.2-1509 of the Code of Virginia.
2. The center's policy states that staff are to "report if an act is witnessed or if there is reasonable suspicion. Caregivers must immediately report any instances or suspicion of child abuse, neglect, lack of supervision, improper treatment towards children, any type of discipline that is not in accordance with policy, etc. that happens on school property to their immediate supervisor."
3. Two staff observed Staff #1 spank Child #2 on 07/09/2024.
4. Staff did not report this observation to their immediate supervisor until 07/10/2024,
Plan of Correction: As soon as we were notified of this incident we removed the individual from the building and terminated the individual.
Standard 8VAC20-780-410-1
Based on staff statements, forbidden actions was used with two children.
Evidence:
1. On 07/09/2024 and 07/10/2024, two staff observed Staff #1 spank a child #1 (age15 months).
2, On 07/10/2024, two staff observed Staff #1 grab child #2 and lift the child up by one arm and move the child.
3. On 07/10/2024, while outside on the playground, a staff observed Staff #1 spank Child #1 when they were trying to push through the gate of the playground.
Plan of Correction: We reported this incident when we learned about it. We connected with the agencies on this matter immediately. We are re-training our staff on mandated reporting, forbidden actions and positive behavior. We will have Loudoun County come to our P.D.D. to do a mandated reporting training.
