Inspector Notes

An unannounced inspection was initiated on 01/29/2025 and completed on 02/11/2025 in response to a complaint received by the licensing office on 01/23/2025 relating to allegations of staffing, supervision and program. The inspector reviewed 2 children?s records and 1 staff records on-site. This inspection included document review, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 02/18/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight supervision of staff.

1. During nap time on 01/29/2025, Staff #1 was observed using their cell phone and was sitting in an area of the classroom which allowed at least 2 children out of sight supervision.

2. Seven out of the 19 children in the classroom remained awake. One was observed laying on the floor and not on a cot. This child was one of the two children that were not within Staff #1's sight supervision.
Plan of Correction: Staff #1 is no longer employed with the school. On 2/17/25 a thorough retraining of our Cell Phone Policy and Staffing and Supervision Policy will be delivered to all employees. School admin will conduct regular classroom observations.

The following actions or threats thereof are forbidden: forcing rest.

1. Staff #1 admitted to repeatedly threatening Child #2 of laying on top of the child if the child did not lay down on a cot to nap.
2. Staff #1 admitted to laying on top of Child #1 while Child #1 was crying to force Child #1 to remain on the cot during nap time.
Plan of Correction: Staff #1 was immediately removed from the class and is no longer employed with the school. On 2/17/25 all staff members will be retrained on Forbidden
Actions and positive behavior support techniques to ensure CBA standards are met. School admin will conduct ongoing classroom observations.
