During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 05/01/2025, as a part of the licensure period. The on-site inspection began at 12:30p.m. and ended at 2:30p.m. The inspector reviewed compliance in the areas listed above. There were 99 children present and 12 staff. The inspector reviewed 8 children?s records and 8 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-440-E
Repeat
There shall be at least 12 inches of space between occupied cots.

A minimum of 12 inches between occupied cots were observed in 2 classrooms.
Plan of Correction: Cots were re-arranged to ensure a 12 inches
of space between cots. Staff received immediate
retraining on cot arrangement requirements.
Management has retrained all staff on proper cot
spacing during naptime. Management will check in
with staff during naptime to ensure cots are
properly spaced regulary.
Standard 8VAC20-780-500-B
The diapering area shall have for children younger than three years, this surface shall be a changing table or countertop designated for changing.

A changing table or countertop was not found in a classroom with children under the age of 3 years.
Plan of Correction: A diaper surface/table will be ordered and installed for this classroom.
Standard 8VAC20-780-510-E
The center's procedures for administering medication shall include methods to prevent use of expired medications.

There were two expired medications found. 1 medication expired 5 days ago and a second medication expired a month ago.
Plan of Correction: These medications will be returned to the parents.
