Inspector Notes

An unannounced monitoring inspection was conducted on April 4, 2024, from 10:00am-11:15 am. At the time of entrance, ( 12 ) children ( 26) points were in care with ( 2) caregivers present. The inspector reviewed 6 children?s records and 3 caregiver/household member records. Children were observed during handwashing and snack time, morning meeting, puzzles, manipulatives and Yoga. One medication was on site but not being administered. Please have the parent take home the medication or complete the proper medication forms to keep the medication at the home. Areas of non-compliance are identified on the violation notice.

Based on an interview with the provider, the family day home has not maintained a written record of children in attendance each day.
Evidence: There was no record of attendance for today or the past couple weeks.
Plan of Correction: The provider will maintain daily attendance.

Based on a review of children records, 1 child's record does not have in writing from the parents whether or not there is insurance liability.
Evidence: Child # 1, date of first attendance 11/30/2020, does not have in writing from the parents the insurance liability form.
Plan of Correction: The parents will complete the insurance liability form.
