Inspector Notes

A non-mandated monitoring was conducted on 3/13/2024 in response to an injury report requiring outside medical treatment received by the office on 3/7/24. On 3/5/2024 a child, age 4, was reported to have fallen over the slide and landed on the ground hurting their arm. A parent later reported the injury was a break in the left arm. The inspection was concluded on 3/22/2024.

The inspector was on site from 1:40pm until 2:45pm. One children's records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on observation, the center failed to ensure that all climbable playground equipment had the required resilient surfacing. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.

Evidence:
On 3/13/2024, observed a non-measurable amount of resilient surfacing under the required fall zone in front of the larger slide of the climbable playset. On the side of the slide, there was approximately 3 inches of resilient surfacing. The required amount is 6 inches of resilient surfacing.
Plan of Correction: Mulch was added on 3/14/2024.
