Inspector Notes

An unannounced monitoring inspection was initiated on 11/4/2024 and completed on 11/12/2024 in response to a self-report received by the licensing office on 10/31/2024 relating to an injury of a child requiring outside medical treatment. The inspector reviewed 1 child?s records and 3 staff records on site on 11/4/2024. This inspection included, document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children.

The screening for Staff #1 was completed two weeks after hire.
Plan of Correction: HR will ensure that all staff have TB screenings at the time of employment or prior to coming in contact with children.

There was a maximum of 6 inches of resilient surfacing under the 7 foot playground equipment. 9 inches of resilient surfacing would be needed
