During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An announced monitoring inspection was initiated on 1/10/2025 and completed on 3/5/2025 in response to a self-report received by the licensing office on 12/18/2024 relating to staffing and supervision. An on-site inspection was conducted on 2/24/2025 with local agencies. The inspection included document review, a tour of the facility, interviews, and observations. The inspector reviewed 4 children?s records and 1 staff record on-site on 2/26/2025.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/12/2025. A POC submitted after this date will not appear on the public website.
Violations
6The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #4, enrolled over 30 days, did not have documentation of an immunization record on file.
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was no physical examination of Child #2 who has been enrolled for over a year.
There was no physical examination of Child #4 who has been enrolled for over 30 days.
Staff 1 did not ensure care, protection, and guidance while supervising a group of children (ages 6-12) during a period of time over a month. There were multiple occurrences of the group of children being left unsupervised in a separate classroom. While being unsupervised, a 10-year-old child had multiple incidents of unwelcomed, inappropriate physical actions with 3 school-age children (ages 7-9).
Staff and children indicated that children (ages 6-12) were alone and unsupervised in a separate classroom on multiple occasions during a period of time over a month. During the time when there was no sight and sound supervision of the children, several inappropriate physical acts occurred with three children (ages 7-9).
Centers shall be in compliance with the center's own policies and procedures required by these standards.
The center?s policy is that children shall remain under supervision of staff at all times. Staff and children indicated that a group of children (ages 6-12) were, on multiple occasions, playing alone and unsupervised in a separate classroom during a period of time over a month.
Centers must keep a separate record for each child enrolled that contains all of the required information.
The record for Child 2, enrolled for over a year, was missing proof of ID.
The record for Child 4, enrolled over 30 days, was not available onsite. A minimum of 16 required components were missing.
