During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced complaint inspection was initiated on 7/03/2025 and completed on 7/21/2025 (with a local agency), and a second licensing inspector, in response to a complaint received by the licensing office on 6/30/3035 relating to allegations of forbidden actions and special services. The Department issued a Special Order on 5/22/2025 to prohibit new admissions of school age children for a period of 30 days and to require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations, as a result of high-risk violations. The inspector reviewed 2 children?s records and 4 staff records on-site and electronically on 7/10/2025. This inspection included document review, a tour of the facility, interviews, and observations
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Though not a part of the original complaint, violations that were not reported were found related to supervision and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/28/2025. A POC submitted after this date will not appear on the public website.
Violations
3The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #1, who had been attending for over a year, had documentation of immunizations on file that were 2 weeks past the needed timeframe.
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
7/03/2025, Staff put three children (age 2) inside the classroom from off the playground. The door to the classroom was shut and there was no sight or sound supervision of the children inside for approximately 1 minute.
07/17/2024, Staff #4 held Child #3 (age 22 months) by one ankle upside down and walked them across the classroom. Child #3 was crying while being held upside down.
