During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/08/2025 as part of the provisional licensure period. The Department issued a Special Order on 5/22/2025 to prohibit new admissions of school age children for a period of 30 days and to require the licensee to contact parent, guardians, or other responsible persons regarding health and safety violations, as a result of high-risk violations. The inspector reviewed 5 children?s records and 3 staff records on-site on 7/8/2025. This inspection included document review, tour of the facility, interviews, and observations.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/18/2025. A POC submitted after this date will not appear on the public website.

Repeat violation
Physical examinations completed by a physician shall be obtained prior to first day of attendance or within 30 days after the first day of attendance.

Child #1, who had been attending for 4 months, had a physical on record that was two months past the required timeframe.
Plan of Correction: All children files have been gone through to ensure that they have the correct information in them.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #1 completed the course two weeks late.
Plan of Correction: HR and PD will ensure that the VDOE training is completed within the 90 days of hire.

When food is brought from home the food container must be clearly dated and identify whom it belongs too.

There were two lunchboxes in the cubbies in the afterschool room. These lunchboxes were not labeled with the child's name and date.
Plan of Correction: he PD has discussed with the After-School teachers that all outside food must be labeled.

Tables shall be sanitized before and after each use for feeding.

Tables that were used for lunch were not cleaned before the children were served food.
Plan of Correction: PD will do a training for teachers to ensure that tables are sanitized before and after each meal.
