During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 7/10/2025 and completed on 08/13/2025 with a local agency and a second licensing inspector, in response to a self-report received by the licensing office on 6/27/2025 related to supervision. The Department issued a Special Order on 5/22/2025 to prohibit new admissions of school age children for a period of 30 days and to require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations, as a result of high-risk violations. The inspector reviewed 1 child record and 1 staff record on-site. This inspection included document review, a tour of the facility, interviews, and observation.
After review by the Licensing Administrator, the inspection documents were updated on 10/29/2025.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/20/2025. A POC submitted after this date will not appear on the public website.
Violations
5The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child 1, who had been attending for over a year, had documentation of immunizations on file that were a year past the needed timeframe.
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was no physical examination in the record of Child 1, who had been enrolled for over a year.
When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 6/27/2025, Child 1 (age 5) left the center unsupervised. The child walked alone beside a busy four-lane road (speed limit 45 mph) with no pedestrian sidewalk.The road connects to two major intersections where multiple cars and semitrucks travel. The child was discovered by construction workers at a construction site about 0.2 miles away from the center. The workers contacted law enforcement about the unsupervised child. Law enforcement officers returned the child to the center. Child 1 was unsupervised by center staff approximately 15-20 minutes.
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
On 6/27/2025, Child 1 (age 5) walked away from the center, down a busy road and into a construction area. Center staff did not have sight and sound supervision of Child 1 for approximately 15-20 minutes.
Centers shall be in compliance with the center?s own policies and procedures required by these standards.
The center?s policy is that children shall remain under supervision of staff at all times.
Child 1 (age 5) left the center and walked down a busy road alone and unsupervised for approximately 15-20 minutes.
