During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/12/2024. The on-site inspection began at 11:30am and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 25 children present and seven staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the date of the previous screening. Staff #5?s TB screening has been overdue for more than three months.
Plan of Correction: This was an over sight on our part. The staff member will schedule and complete the TB screening as soon as possible.
Standard 8VAC20-780-550-H
The center shall maintain a record of the dates of the practice drills for one year. The drill log did not document information for the evacuation drill completed in November 2024. Management stated the evacuation drill was completed but was not written down on the drill log.
Plan of Correction: Going forward, we will ensure all emergency drills are documented on the official drill log.
