Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 04/21/2025. The on-site inspection began at 2:00pm and ended at 4:30pm. The inspector reviewed compliance in the areas listed above. There were 54 children present and eight staff. The inspector reviewed five children?s records and four staff records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #2 has been attending the center for two years and seven months. The record for Child #2 contained one original immunization record from when the child first enrolled at the center at the age of four months.
Plan of Correction: I will request the information from the parents and I will add the information to the child's record.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There was a hole approximately two inches in diameter on the wall in the school age bathroom.
Plan of Correction: We will patch the hole in the wall.
