During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 3/11/2025, as a part of the licensure period, The on-site inspection began at 11:15 am and ended at 1:45 pm. The inspector reviewed compliance in the areas listed above. There were 125 children present and 25 staff. The inspector reviewed 4 children?s records and 4 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Room 2 had a broken blind and exposed rusted pipe on back of child toilet.
Plan of Correction: We are having blinds replaced throughout the whole center.
Standard 8VAC20-780-440-F
Rest mats are used, and shall have cushioning and be sanitized on all sides weekly or before use by another child.

Three rest mats in room 5 had visible tears preventing them from being sanitized as required.
Plan of Correction: This is already on the list to replace.
Standard 8VAC20-780-50-A
Staff and children's records shall be treated confidentially.

Menu with child name and allergy were hanging outside the door with out a cover in classroom 9
Plan of Correction: They will be moved and a cover added.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include required information.

6 of 6 accident reports did not have method of notification on report.
Plan of Correction: There are new forms being created. We will add required information to form.
