Inspector Notes

An unannounced monitoring inspection was conducted 3/28/2024 at 11:08am. There were 22 (2-12yrs) directly supervised by 5 staff. The physical plant, 3 staff records, 4 children?s records, 4 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing interactive games, watching a move, and eating lunch. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard 22.1-289.035-A
Based on review, the facility did not ensure that employees obtain subsequent background checks every five years.
Evidence: reviewed 2 staff records and found that Staff 2's last central registry check was dated 3/25/2018.
Plan of Correction: 1) Will submit new (CRS) for (staff) since previous updated forms have not been returned.
2) Will keep a phone log/communication log along with printed receipts to show when request where made due to back log.
Standard 8VAC20-780-280-H
Based on observation, the facility did not ensure that cosmetics, medications, or other harmful agents shall not be stored in areas, purses or pockets that are accessible to children.
Evidence: observed that there were staff multivitamins stored in a teacher drawer accessible to children.
Plan of Correction: Multivitamin was removed 3/28/2024 immediately.
Standard 8VAC20-780-560-G
Based on observation, the facility did not ensure that when food is brought from home the container is labeled and identifies the owner.
Evidence: observed that there were no dates on the lunchboxes in the Caterpillars and school-age classroom.
Plan of Correction: Reusable tags were created. Tags include children's first and last name with a space for the date. 4/1/2024

Based on review, the facility did not ensure that children's records were maintained with the required minimum information.
Evidence: reviewed 4 children's records and found that Child 2 did not have documentation of the parent's annual review of emergency contact information. The last documented review was dated 12/18/22.
Plan of Correction: Child 2's parents have signed and updated forms. 4/2/2024
Standard 8VAC20-780-80-A
Based on review, the facility did not ensure that the attendance record documented the arrival and departure of each child in care as it occurs.
Evidence: reviewed the attendance records in both rooms and found that the Caterpillars room had 4 children documented on their attendance log and there were 8 children in attendance.
Plan of Correction: Remind staff to record attendance immediately regardless of activity (i.e. Easter Bunny visit)
