Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to breakage of any equipment and head entrapment hazards.
The Director acknowledged the slide component of the playground structure has been resecured with a caulking material that is currently separating. The mesh net installed near the slide area also poses a potential head entrapment risk for children.

Plan of Correction: I will notify the church to correct the violations on the climber. Our students will not use the apparatus until it is up to code.

In Room #1 three out of the four children's lunchboxes and water bottles were not labeled.
