Inspector Notes

An unannounced monitoring inspection was conducted on May 15, 2023 from approximately 11:45 am to 12:45 pm. There were 37 children in attendance and a total of 13 staff present. 4 staff records and 4 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed getting ready for lunch, playing with blocks, and playing outside while the licensing inspector was on-site.

The most recent health inspection was conducted on 06/17/22. The most recent fire inspection was conducted on 8/25/22. Evacuation drills were conducted monthly with the most recent on 04/27/23.

Violations were cited as a result of this inspection.

Based on records review, and interview, the center failed to ensure that staff shall complete annually a minimum of 16 hours of training. Evidence: 4 out of 4 staff records reviewed did not have any training hours for the previous year. When asked the Director stated that she did not realize staff had to complete 16 hours of training annually.

Based on records review, the center failed to obtain a parent agreement that is signed by the parent by the first day of the child?s attendance. Evidence: The record for child #3, date of enrollment 09/13/21, did not contain a signed parent agreement.
