Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 12/16/2021 and concluded on same. The director was contacted by telephone and a virtual inspection was conducted. There were 32 children present, ranging in ages from 5 months to 5years. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child records and 1 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Child #1 has an Avi-Q auto-injector at the facility. The medication is not in an original container with a prescription label attached.

Staff A's record did not contain documentation of current First aid and CPR certification. The certification on file expired 6/2021. Recertification class was not taken until October 2021: however, there is no documentation to show the certification is current.
