Inspector Notes

An in-person renewal study was conducted today. All rooms were in ration during the inspection. Rooms were remeasured and measured to add all rooms to the floor plan.
2 staff and 4 child records were reviewed. See Violation Notice for missing items. Emergency drills, emergency supplies, first aid supplies, menu, rest cot spacing, infant bottles, playground and vehicles are all in compliance. Board Officer Background Checks are all in compliance. The inspection will be typed off site and emailed to the facility for signatures. LI was present today from 1:10 PM-4:10PM. Based on a review of records, immunizations for children under 2 have not been updated every 6 months.
Evidence:
1 of 4 child records reviewed did not contain an updated immunization record every 6 months for s child under 2 years of age. Child 1 (Date of Birth 12/18/2020) does not have an updated record for 12/2021 on file. The last immunizations available were dated 6/2021
Plan of Correction: The parent will provide an update health record today.

Based on a review of records, one staff record does not contain documentation of a repeat TB screening within the allowable 2 year time frame.
Evidence:
1 of 2 records reviewed did not contain a repeat TB negative screening result within 2 years.
Staff A's date of screening is documented as 11/6/2019 (expired).
Plan of Correction: Staff person will go get a screening or test tomorrow.
