Inspector Notes

An in person monitoring inspection was conducted today.
All rooms were in ratio during the inspection. Children were observed washing hands, eating lunch, preparing for rest time, exercising, and sleeping quietly.
1 medication was reviewed. Child cot spacing was correct in the Penguins room.
Emergency evacuation drills, shelter-in-place drills and lock down drills were all in compliance.
3 child and 2 staff records were reviewed, injury reports, See Violation Notice for non-compliances.

Standard 8VAC20-780-420-E-1
Based on staff statement and a review of available documentation, one child that requires time spent on their stomach, has not had 30 minutes of "tummy time" documented daily.
Evidence:
No documentation was available for one child, showing- 30 minutes of required tummy time on the child's daily sheet for 10/03/2022. Staff stated it was not written down. (Child 1)
Plan of Correction: Infant had tummy time staff had not documented. Reminder to all staff to document in real time.
Standard 8VAC20-780-420-E-3
Based on a review of records, the center staff did not request at least annually parent confirmation that the required information in the child's record is up to date.

Evidence:
2 of 3 child records reviewed did not contain documentation of an annual review and update.
Child 2's record is dated 5/21/21 (expired)
Child 3's record is dated 9/13/21 (expired)
Plan of Correction: All registration forms are being distributes and Laurel Learning is requiring all families to have completed registration packets returned by 10/31/22

Based on observation and staff statement, cribs in use were not labeled for use by a specific child.
Evidence:
2 children were sleeping in cribs on the younger infant side of the infant suite, in cribs that were not labeled with their names. (Children #4, 5)
Plan of Correction: Cribs were labeled. All cribs have names.

Based on a review of available documentation, injury reports do not contain documentation of all required information.
Evidence:
5 of 6 injury reports reviewed did not contain documentation of the time and date of parent notification, or the method of notification.
Plan of Correction: Meeting to refresh staff and admin team how to complete injury reports. All reports were reprinted highlighting time/date of parent notification.
