During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on April 22, 2025. The on-site inspection began at 10:40am and ended at 1:25pm. The inspector reviewed compliance in the areas listed above. There were 43 children present and 11 staff. The inspector reviewed seven children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 10/10//2024, were not posted in a conspicuous place on the premises.
Plan of Correction: The director will ensure the most recent inspection reports are posted. The last inspection report, dated 10/10/24 was posted during the inspection.
Standard 22.1-289.057-D
Programs can choose to use bottled water, water coolers, or other similar water sources for human consumption that meets the U.S. Food and Drug Administration standards for bottled water. Pursuant to ? 32.1-167 of the Code, human consumption ?means drinking, food preparation, dishwashing, bathing, showering, hand washing, teeth brushing, and maintaining oral hygiene.?

The program has chosen, at this time, to use bottled water and was not following the bottled water standards for human consumption.
Plan of Correction: A company was contacted and water is scheduled to be tested on 4/29/25 at 9:00am. Until the water testing is complete, the facility will follow the bottled water requirement.
Standard 8VAC20-780-160-C
Repeat violation: A tuberculosis (TB) screening is required every two years from the last screening. Staff #2, #3, and #4 updated screening was due six months ago. The updated screenings have not yet been completed.
Plan of Correction: The staff will have the tuberculosis (TB) screening completed by Friday, 4/25/25. We will do a better job in monitoring the expiring TB screenings.
Standard 8VAC20-780-245-A
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. Staff #1, #2, #3, #4, and #5 did not complete a minimum of 16 hours of training in 2024.
Plan of Correction: We will do a better job in documenting the training hours starting tomorrow.

After the first license, annual approval from the health department shall be provided. The annual environmental sanitation inspection had not be completed annually.
Plan of Correction: The health inspector was contacted and the inspection will be completed annually going forward
Standard 8VAC20-780-330-B
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. The fall zones on the playgrounds had less than an inch of resilient surfacing of mulch.
Plan of Correction: We are schedule to have additional mulch installed on our playgrounds.
