During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated and completed on July 31, 2025 in response to a self-report received by the licensing office on July 30, 2025 relating to supervision. The inspector reviewed 1 child's record and 2 staff records on-site and electronically on July 31, 2025. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to records and training and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 14, 2025 . A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-2
All applicants for employment must submit to fingerprinting prior to employment.

Staff #2's fingerprinting results were dated over two months after the date of employment.
Plan of Correction: All new employees will be fingerprinted before teaching in the classroom. Staff #2 late fingerprint results was an unfortunate oversight.
Standard 22.1-289.035-B-4
A search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years must be conducted prior to employment.

Staff #2 resided in one state outside of Virginia in the preceding five years. The center did not conduct a search of the child abuse and neglect registry for that state. Staff #2 has been employed for nine months.
Plan of Correction: Laurel conducted an out of state CPS check for staff # 2 that lived outside of Virginia in the last 5 years. CPS results showed no record.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.

Staff #2 did not submit documentation of a TB screening. Staff #2 has been employed for nine months and has been in contact with children.

REPEAT VIOLATION

At least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis (TB) screening.

Staff #1's follow-up TB screening expired seven weeks ago.
Plan of Correction: A gross oversight occurred when Laurel admin forgot to check the TB expiration of summer seasonal employee. Staff #1 and #2 were given TB medical form to be completed.

Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Staff #1 and Staff #2 did not complete orientation training. Staff #1 has been employed for 19 months. Staff #2 has been employed for nine months.
Plan of Correction: Laurel will revisit staff orientation with all staff during staff meeting on August 13, 2025

Prior to working alone with children and within seven days of the first day of employment, staff shall be provided in writing with the center's procedures for identifying where attending children are at all times.

Staff #1 and Staff #2 were not provided in writing with the center's procedures for identifying where attending children are at all times prior to working alone with children and within seven days of the first day of employment. Staff #1 has been employed for 19 months. Staff #2 has been employed for nine months.
Plan of Correction: Laurel has revisited policies and procedures for new staff. We are having a staff meeting August 13, 2025 and will go over center?s policies and procedures in writing for all current staff.

When staff are supervising children, they shall always ensure their care, protection and guidance.

On July 30, 2025, Child #1 (age: 2 years) was left unsupervised on the playground for approximately five minutes. The playground fence does not have a lock and leads out to a parking lot. The parking lot is located next to a street with a speed limit of 25 mph. The playground is also surrounded by an accessible wooded area.
Plan of Correction: Director met with the staff to retrain and emphasize the importance of head counting and using the classroom Passport roster. The passport roster should be checked and triple checked especially during transition to and from playground or any time away from the classroom.

In each grouping of children at least one staff member who meets the qualifications of a program leader shall be regularly present.

On July 30, 2025, Staff #1 and Staff #2 worked together in a classroom with no other staff.
Staff #1 and Staff #2 do not have on record documentation showing that they meet the qualifications of a program leader. Staff #3 confirmed that Staff #1 and Staff #2 did not have the qualifications of a program leader.
Plan of Correction: Last minute absence of both classroom teachers left admin scrambling for staff. Admin completely forgot the 2 staff subs were not qualified lead teachers. We will make sure at least one lead staff member is always in a classroom

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

At least one staff in each classroom or area where children are present shall have current certification in cardiopulmonary resuscitation (CPR) and first aid.

On July 30, 2025, Staff #1 and Staff #2 worked together in a classroom with no other staff. Staff #1 and Staff #2 do not have current certification in CPR and first aid.
Plan of Correction: Staff #2 will have CPR/1st Aid training in September 2025. Staff#1 is returning to college and will have CPR/1st Aid training before she returns as a seasonal summer employee. Director and admin team will remember to make sure at least one staff member is trained in CPR/1St Aid when teaching in a classroom.

Documentation that two or more references as to character and reputation as well as competency were checked before employment shall be kept for each staff person.

There was no documentation that references were checked for Staff #1 and Staff #2. Staff #1 has been employed for 19 months. Staff #2 has been employed for nine months.
Plan of Correction: Admin neglected to obtain referrals for staff. On August 4, 2025 Laurel hired an Administrative Assist to assist the Director with employee documents. Laurel is obtaining referrals for all employees if they don?t have any.
Standard 8VAC20-780-80-A
REPEAT VIOLATION

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Classroom #1's attendance record for July 30, 2025 did not document the arrival and departure of Child #2 and Child #3 as it occurred.
Plan of Correction: Staff had the passport roster but failed to add the names of the children that were transitioning into their new classroom. Though the transition was only for 11/2 hours names of any new child in the classroom needs to be added to the passport. The 2 staff were subbing in the classroom because both classroom teachers were unfortunately absent.
