During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on September 15, 2025 and completed on September 24, 2025 in response to a complaint received by the licensing office on September 12, 2025 relating to supervision. The inspector reviewed 1 child's record and 2 staff records on-site on September 15, 2025. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/1/2025 . A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION

When staff are supervising children, they shall always ensure their care, protection, and guidance:

Live photo evidence shows child #1 (age: 4 years) was laying on the floor alone crying while unsupervised in the hallway at 12:03PM on February 5, 2025 outside of Classroom #1. The child was alone for an unknown amount of time. There is no staff visible in the photo. Classroom #1 teachers did not ensure care protection and guidance when leaving child #1 laying on the floor unsupervised in the hallway.

The operational responsibilities of the licensee shall include ensuring
that the center's activities, services, and facilities are maintained in
compliance with these standards, and the center's own policies and
procedures.

The center has a discipline policy as required by the standards that states, "We believe that is our responsibility to provide children with positive guidance."

Live photo evidence shows child #1 was in the hallway outside of Classroom #1 alone while crying on the floor. It was reported by a parent that child #1?s shoes were thrown at the child from the classroom into the hallway on more than one occassion.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts:

Live photo evidence shows child #1 (age: 4 years) left alone laying on the floor in the hallway while crying on February 5, 2025. There is no staff supervision observed. There is no evidence that staff attempted to redirect the child to the appropriate behavior.
