Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on April 16, 2021 and concluded on April 19, 2021. The director was contacted by telephone to initiate the inspection. There were 81 children present with 19 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed four children's records and seven staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Standard 22VAC40-185-60-A
Based on review of children's records, the center failed to ensure all required information was in children's records.

Evidence:
1. Child #1 and Child #3 were missing a complete emergency contact address for one out of two emergency contacts.
2.Staff verified that there was not a complete emergency contact address for Child #1 and that one of the emergency contacts for Child #3 was missing the house number.
Plan of Correction: Administration will contact parents today to get the complete address for both emergency contacts. When a child is enrolled into the program the forms will be reviewed to ensure all required information is provided.
