During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 10/1/2025 and completed on 10/1/2025, as a part of the licensure period. The on-site inspection began at 11:58am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were eight children present and three caregivers; the point total was twenty-six. The inspector reviewed five children?s records and four caregiver/household members records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

REPEAT VIOLATION:
A tuberculosis (TB) screening is required every two years from the last screening.

Updated TB screenings for were not completed and were overdue for Caregiver A and Household Member #1 by eighteen days and for Caregiver B by twenty-three days.
Plan of Correction: Will complete the updated TB screenings.
Standard 8VAC20-800-550
A play pen must not be used for the designated sleeping area.

Child #4 was sleeping in a play pen.
Plan of Correction: Will provide a crib, cot, or rest mat for resting or napping.

REPEAT VIOLATION:
Children's records are required to contain parental names, parental home and employment addresses and phone numbers, information for two emergency contacts, the policy number of the child's medical insurance, and written authorization for emergency medical care.

The record for Child #5 was missing the parental names, their home and employment addresses and phone numbers, information for two emergency contacts, the policy number of the child's medical insurance, and written authorization for emergency medical care. The child was present during the inspection and had been in care for ten months.
Plan of Correction: Will obtain the missing document and information from the parent.
Standard 8VAC20-800-80-A
Within seven business days of the child's first day of attendance, the provider shall obtain from the parent verification of the identity and age of the child and name and location of previous day care programs.

This information was not available for Child #5, who has been enrolled for 10 months.
Plan of Correction: Will obtain from parents and complete the proof of identity form.
