Inspector Notes

A renewal inspection was initiated on 10/31/2022 and concluded on 10/31/2022. There were 5 children present with the provider. The point total was 10. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 1 provider record was reviewed. The inspection started at 9:00am and concluded at 10:30am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on review of records the provider failed to maintain compliance ensuring that repeat central registry clearances are completed every five years as required.

Evidence:
The central registry clearance for the provider had not been renewed as required. The clearance in the file was dated 6/10/2017. These are to be renewed every 5 years.
Plan of Correction: A request for renewal had been sent, but nothing has been received to date. During the inspection today another request for an updated clearance was submitted through the portal.
