Inspector Notes

An unannounced, on-site renewal inspection was initiated on 11/04/24 and completed on 11/04/24. The on-site inspection began at 8:45am and ended at 10:00am. The inspector reviewed compliance in the areas listed above. There was 1 child present and 1 caregiver; the point total was 2. The inspector reviewed 5 children?s records and 1 caregiver record on-site. This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.036-A
Every applicant for licensure as a family day home, at the time of application, who is or will be involved in the day-to-day operations of the family day home, or who is or will be alone with, in control of, or supervising one or more of the children shall undergo a background check prior to issuance of a license as a family day home, and every five years thereafter.

The criminal background check for the caregiver had expired over 6 months ago and had not been renewed as of this inspection.
Plan of Correction: I will immediately obtain the required fingerprints now that I have the code that I have been trying to obtain for over 10 months.
Standard 8VAC20-800-140-B-3
Current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.

The caregivers CPR certification had expired as of September 2024.
Plan of Correction: A CPR course will be completed as soon as possible.

Current certification in first aid.

The caregivers First Aid certification had expired as of September 2024.
Plan of Correction: A FA course will be completed as soon as possible.

The provider shall obtain for each caregiver a current Report of Tuberculosis Screening form every two years from the date of the first screening.

The TB screening for the caregiver had expired over 9 months ago and had not been renewed at the time of this inspection.
Plan of Correction: A new TB screening will be completed as soon as possible.
