Inspector Notes

An unannounced monitoring inspection was conducted May 8, 2024 approximately 3:35 pm to 4:30 pm. There were 19 children directly supervised by 6 staff. The physical plant, 2 staff records, 2 children?s records, medications, evacuation drills, emergency supplies were inspected. Children were observed participating in group play, reading books, and handwashing. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance.

Based on staff reords, the center did not maintain doucmentation at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
Staff 1 tuberculosis screeening expired on 8/15/2023.
Plan of Correction: Staff #1 is scheduled to take tuberculosis test, Other staff files reviewed and any 2024-2025 staff expirations will preemptively recertify before start of 2024-2025 school year with updates to records taking place immediately thereafter
