During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

The Department requested that the provider complete an Intensive Plan of Correction (IPOC) as result of the last inspection related to record keeping and background checks.

Inspector Notes

An unannounced monitoring inspection was initiated on 7/21/2025 and completed on 9/8/2025 with the local DSS agency in response to a self-report received by the licensing inspector on 7/21/2025 relating to supervision and the care and protection of children. The on-site inspection began at 11:05am and ended at 2:40pm. The inspector reviewed 1 child record and 3 staff records on-site. The inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not part of the original self-report, violations that were not reported were found related to record keeping and background checks and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be September 16, 2025. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION
The center is required to obtain a completed sworn disclosure statement prior to the employee's first day of employment.

On the date of the inspection, the record for Staff #2, employed for more than 30 days, and Staff #3, employed for almost 30 days, did not contain a sworn disclosure statement.

REPEAT VIOLATION
Each staff member shall submit documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician.

The record for Staff #2 did not contain documentation of a TB screening.
The record for Staff #3 contained a TB questionnaire that was dated 5 months prior to hire and not signed by a physician.

Staff shall always ensure the care, protection, and guidance of children.

On 7/18/2025, a 5 year old child that was attending summer camp was left unattended on the community playground for approximately 10-15 minutes. Center staff were unaware the child was left behind, resulting in an unknown father returning the child to a custodial worker.

Multiple staff interviews revealed Child #1, age 5 years, was not within actual sight and sound supervision for approximately 10-15 minutes.. Child #1 was left outside on the playground while the rest of the group went inside.

The operational responsibilities of the licensee shall include ensuring that the centers activities, services, and facilities are maintained in compliance with the center's own policies and procedures.

Staff #1, #2, and #3 failed to follow the centers Name-to-Face procedures on 7/18/2025 when returning from the playground which resulted in a 5 year old child being left outside and unattended for approximately 10-15 minutes.
