During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

The inspection was amended on 07/25/2025 and sent to provider on 07/25/2025

An unannounced, on-site monitoring inspection was initiated on 07/17/2025 and completed on 07/17/2025. The on-site inspection began at 9:22am and ended at 10:52am. The inspector reviewed compliance in the areas listed above. There were seven children present and one caregiver; the point total was 14. The inspector reviewed five children?s records and two caregiver records on-site. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/24/2025. A POC submitted after this date will not appear on the public website.

Caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Caregiver #1, employed for two years, only had 1.5 hours documented for 2024.
Plan of Correction: Have employee #1 take training classes
Standard 8VAC20-800-350-A
Hot water at taps available to children shall be maintained within a range of 105?F to 120?F.

The water from the sink where children wash their hands was measured at 126?F
Plan of Correction: Turn temperature down lower. Reach out to new installer again.

A working battery-operated flashlight, a working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times.

There was no battery-operated flashlight at the time of inspection.
Plan of Correction: Flashlight bought and hung. Extra batteries already on hand. Leave flashlight hanging.
