Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on May 7, 2021 and concluded on May 11, 2021. The director was contacted by telephone to initiate the inspection. There were 3 children and 4 staff present during the remote inspection as the program is currently closed. However, the program offers distance learning. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations
1Evidence: In Staff#2's record (date of hire: 1/22/19) the TB results last dated was 12/24/18 and there was no updated TB results found. Interview with staff confirmed updated TB results were not available for review during this inspection.
A second notice will be sent to the staff his/her supervisor, and the Director after 30 days, if not received. If the staff unable to see his/her PCP the agency will refer staff to I&O Medical to expedite the process.
The Health Coordinator will sent a digital copy of the result to HR to file.
