The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 6/24/2025, were not posted in a conspicuous place on the premises.

The center shall ensure staff have a tuberculosis screening that is signed by a physician, the physician's designee or an official of the local health department. The record for Staff #2 had a tuberculosis screening that was not signed and has been employed for seven months.

(Repeat violation) Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #1, who is working with children, has not yet completed this course which is overdue by five months. Staff #3, who is working with children, has not yet completed this course which is overdue by two months.

The center shall ensure there is annual approval from the health department. The most recent health inspection available for review was completed two years ago.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1 did not have documentation of health information as required.

(Repeat violation) The center shall maintain a record for each staff with required documentation. The record for Staff #1 did not have documentation to demonstrate the education and training required by the job position.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. In Huddle 1, twenty-seven children were observed in the group however twenty-one children were marked present on the attendance record. In Huddle 3, seventeen children were observed in the group however fourteen were marked present on the attendance record.
