Inspector Notes
An unannounced renewal inspection was conducted on 3/9/2023 from 12:30 pm-2:45 pm. There were 88 children supervised by 19 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, and preparing for a nap. Hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations
4Evidence:
1. The building was built prior to 2015 and the center currently provides care for children who are preschool-age.
2. The Director contacted a facilities administrator to confirm whether the center had a carbon monoxide detector, and it was discovered that the center did not have one.
Evidence: The center did not have documentation of the lockdown drill procedure being practiced in 2022.
Evidence: Child #3 and child #5 are missing proof of identity.
*Date is based on a search of how long it takes to get a passport which states 8-11 weeks.
Evidence: There was no documentation of a written care plan for child #3 and child #4, who both have a diagnosed food allergy.
