Inspector Notes
A complaint investigation concerning staffing and supervision was initiated on 1/11/2024, from a report received by the Fairfax County Licensing office on 1/9/2024. The investigation was conducted on 1/11/2024, in the afternoon between 4:10 pm and 6:00 pm, and completed on 1/18/2024. The investigation included interviews with the administration and staff. The preponderance of evidence does support the allegation and the complaint has been deemed valid. For violation details, please refer to the Violation Notice. Based on review of attendance records and staff interviews, the center failed to ensure that staff-to-child ratios are required whenever children are in care.
Evidence:
1. The attendance sheets for the Academy classroom (Limes, Pineapples, Mosaic, and Oakton) on Friday, December 8, 2023, documented the group being over ratio during the following approximate times: 4:00 pm-4:20 pm and 4:30-4:45 pm. There were approximately 19-22 children between the ages of 5 years old to 8 years old with 1 staff exceeding the 1:18 ratio.

The attendance sheets for the Academy classroom (Limes, Pineapples, Mosaic, and Oakton) on Friday, January 5, 2024, documented the group being over ratio during the following approximate times: 4:00 pm-5:00 pm. There were approximately 21-25 children between the ages of 5 years old to 8 years old with 1 staff exceeding the 1:18 ratio.

Plan of Correction: We will put another staff member in at 4 pm and keep that staff member there until the ratio is at 1:18.
