Inspector Notes

A renewal inspection was conducted on January 3, 2023. There were ten children present with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review, provider did not ensure 1 of 3 children?s records has a physical examination prior to the child?s attendance or within 30 days after the first date of attendance.

Evidence: The record for child #2 (start date 9-20-2022) did not have a physical exam.

Based on observation, provider did not ensure bathroom has required supplies.

Evidence: There was not paper towels in the hall bathroom used by children.
Plan of Correction: Paper towels will be purchased and kept in the bathroom.
