Inspector Notes

A renewal inspection was conducted on April 21, 2022. There were six children present, ranging in ages from infants to five years, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records, two caregiver records and one household member record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-240-A
Based on observation, provider did not ensure areas and furnishings of the home, inside and outside, are maintained in clean, safe and operable condition.

The following was observed on the outside swing playset:
1. The chains for the individual swings were wrapped around the playset top beam rather than connected by the S-hooks.
2. One of two of the bench seats in the multiple-child glider swing on the playset was not connected causing the seat to fall on one side.
Plan of Correction: Provider stated older children, not enrolled in the program, recently did this to the playset. Those children will not be allowed to play on the playset again. The playset will be repaired over the weekend.
Standard 8VAC20-800-430-A
Based on observation, provider did not ensure children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision do not have access to stairs.

Evidence: The gate at the bottom of the stairway leading to the upstairs was propped against the wall. The stairway is in the hallway by the children's "classroom" and the hall bathroom that is used by children. Children in care ranged in age from infants through four years old.
Plan of Correction: The gate will be properly installed and kept closed.
Standard 8VAC20-800-610-A
Based on observation, provider did not ensure use of media is limited to programs, tapes, websites, and software that are produced for children or are suitable for children.

Evidence: At approximately 10:15 AM, provider was present with 4 children in the room designated as the "classroom". A major news network was playing on the television broadcasting updates on international war.
Plan of Correction: The television will only be on when it is programming suitable to children.
