Inspector Notes
This monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person inspection was conducted of the program.
The inspection was initiated on September 27, 2022 and concluded September 27, 2022. There were 6 children present, ranging in ages from 9 months to age 9, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical equipment/ environment, care of children, preventing spread of disease, emergencies and nutrition. A total of 4 child records, I provider and 1 assistant record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program
Violations
9Evidence: The provider confirmed the health examination for child #2 was missing. Child?s first date of attendance was 6-28-2022.
maintain an up-to-date record at the family day home for each enrolled child.
Evidence: The provider confirmed they did not have a record for Child #6, age 9. Provider stated they ?only come for little bit in the afternoon".
Evidence: The provider confirmed the following was missing from the children?s records as listed below;
1. The record for Child #1 did not include written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds.
2. The record for Child #2 did not include the name, address and telephone number of each custodial parent?s place of employment; the name, office address and telephone number of the child?s physician; the emergency contact?s home address; information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation; name and policy number of the child?s medical insurance, names of persons other than the custodial parents who are authorized to pick up the child; chronic physical problems, pertinent developmental information, any special accommodations needed; parent?s signed acknowledgement of the receipt of the information required by to be given to parents prior to child?s first day of attendance.
Evidence: The following infants were observed confined to a space for more than 30 minutes.
1.Child #5, 8 months old was observed in a plastic play yard that measured 4 `9?? by 5?2?? from the time inspector entered the home at 3:10 pm and child #1 was still in the space when the inspection ended at 4:30pm.
2. Child #2, 10 months old was observed in a crib from the time inspector entered the home at 3:10 pm until 4:14 pm when child #1 was removed from the crib for diaper change. At 4:19 pm Child #1 was placed back into the crib and was observed still in the crib when the inspection ended at 4:30 pm.
There were 6 children observed in care during the inspection and there was one provider and one assistant. Provider was observed serving snacking and obtaining items requested by the Licensing Inspector (children?s record, staff records and emergency drill documentation). Assistant gave inspector flashlight, radio and first aid kit to review and then was observed ?putting together new children?s packets? as stated by the provider. The assistant was also observed conducting one diaper change.
Evidence: Child #2 was observed during a diaper change. Immediately after the diaper change child #2 was placed in a crib and the provider did not wash the child?s hands with liquid soap and warm running water as required after diapering or toileting.
Evidence: The provider confirmed the record for Child #3 did not include an updated signature for emergency information as the most recent date was signed was 10-16-2019.
Evidence: The provider confirmed they did not have the telephone number posted for the responsible person for emergency backup care.
Evidence: The provider confirmed there was not an immunization record or information for Child #3. Child #3?s first date of attendance was 10-14-2019.
Evidence: There were two bottles observed on a desk without the infant?s names and dates. Provider confirmed they belonged to two different infants (Child #2 and Child #5).
