Inspector Notes
A subsidy health and safety (SHSI) inspection was initiated and completed on May 3, 2023. There were 9 children present with 2 providers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and vendor interviews, the vendor did not ensue to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: The attendance list was reviewed for the current week and the vendor did not document the departure of children as it occurred. This was confirmed by the vendor.

Standard 8VAC20-790-250-C
Based on observation and vendor interviews, the vendor did not ensue orientation training for all required topics for caregivers shall be completed on the following specific topics prior to the caregiver working alone with children and within seven days of the date of employment or the date of subsidy vendor approval.
Evidence: Vendor confirmed, Provider #2 had not received orientation in the following subjects within 7 days of the provider becoming subsidy. Subsidy effective dated was 2-16-23.
1 Prevention of shaken baby syndrome or abusive head trauma including coping with crying babies and fussy or distraught children
2. Prevention of sudden infant death syndrome and use of safe sleeping practices
3. Caregivers who work with children who have food allergies shall receive
training in preventing exposure to foods to which the child is allergic, preventing
cross contamination and recognizing and responding to any allergic reactions
