Inspector Notes
An unannounced monitoring inspection was conducted on September 27, 2023 from 2:15 PM- 3:45 PM. At the time of entrance, 5 children (age range 7 months to 2 years old) totaling 17 points were in care with 2 providers. The sample size consisted of 5 children?s records, 2 provider records and 2 household member records. Children were observed participating in their rest period and preparing for their afternoon routine.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program
Violations
5Evidence: Provider #1?s record did not include an updated Criminal Records Check (CRC). The most recent CRC is dated 8-29--2018. Provider #1 confirmed they had not yet submitted the CRC.
Evidence: Household member #4 began residing in the home on 4-13-2023 and they did not have a CPS check or CRC .
Based on record review and provider interviews, the provider did not ensue to obtain from each provider and adult household member a current report of Tuberculosis Screening form, in accordance with the requirements, every two years from the date of the first screening. department.
Evidence: The following providers and household members did not have updated TB screenings as required.
1.Household member #3 TB screening was dated 8-27-16.
2.Provider #1 TB screening was dated 3-24-21
3. Provider #2 TB screening was dated 3-24-21
4. Household member #4 TB screening was dated 3-24-15.
Evidence: The provider confirmed the following was missing from the children?s records as listed below;
1.The record for Child #1 and Child #2 did not include the insurance policy number of the child?s medical insurance.
2.The record for Child #1 did not include the employment location and telephone number of each custodial parent?s place of employment.
Evidence: Two out of five children?s records did not contain the written acknowledgment of parents receiving the required information.
