Inspector Notes

An unannounced monitoring inspection was conducted on September 27, 2023 from 2:15 PM- 3:45 PM. At the time of entrance, 5 children (age range 7 months to 2 years old) totaling 17 points were in care with 2 providers. The sample size consisted of 5 children?s records, 2 provider records and 2 household member records. Children were observed participating in their rest period and preparing for their afternoon routine.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Based on record review and provider interviews, the licensee did not ensure to obtain updated background checks as required every 5 years.
Evidence: Provider #1?s record did not include an updated Criminal Records Check (CRC). The most recent CRC is dated 8-29--2018. Provider #1 confirmed they had not yet submitted the CRC.

Based on record review and provider interviews, the provider did not ensure that a person living in the home of a licensed family day home provider had a sworn statement or affirmation when the person age 18 years or older begins residing in the home. A search of central registry (CPS) and criminal history record check (CRC), as requested by the individual must be done within 30 days of a person 18 years of age or older beginning to reside in the home.
Evidence: Household member #4 began residing in the home on 4-13-2023 and they did not have a CPS check or CRC .
