During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 8-25-2025 completed on 8/26/2025. The on-site inspection began at 2:00 pm and ended at 3:10 pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers. The point total was 20. The inspector reviewed 5 children's records and 2 caregiver and 1 household member(s) records on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/5/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
Providers must keep a written record of children in attendance each day. There was only 1 child marked fin attendance and there were 7 children present.
Plan of Correction: Attendance for all children are completed
Standard 8VAC20-800-510-C
Providers must ensure occupied cribs, cots, rest mats, and beds shall be at least 12 inches from each other. There were 2 children observed sleeping in cribs and they were 4 inches apart and 3 children were observed sleeping on cots and they were between 4 and 7 inches apart.

Children's records are required to contain parental employment phone numbers. The record for Child #1 and #4 were missing the employment phone numbers for one parent. The children were present during the inspection and Child#1 had been in care for 14 months and Child #4 had been in care for 11 months.

When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated; given to parents or posted or placed in an area accessible to parents and list any substituted food. There was not a menu available for the current week
