Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A monitoring inspection was initiated on 09/03/2021 and concluded on 09/08/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 9 children, ages 13 months- 3 years, and 3 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and discussion with staff, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence: The record for Child 1 had a documented start date of 05/10/2021. There was not documentation of a physical examination in the record for Child 1 during the 09/08/2021 inspection.
Plan of Correction: Will request the information from the parent.
