Inspector Notes
An unannounced renewal inspection was conducted on 06/11/2024. There were 49 children, ages 7 months- 12 years, and 9 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in the following activities: finishing up breakfast, in free choice activities, participating in an art project, and in outdoor large motor play. Infants were observed being fed and sleeping on demand. A total of 5 children?s records, 5 staff records, 2 applicant records, and 3 medications were reviewed. The inspector arrived for the inspection at 8:15 am and departed at 10:40 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
7Based on record review, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.
Evidence: The record for Staff 3 contained a documented hire date of 10/11/2023. The eligibility letter for Staff 3 was dated 10/13/2023.
Evidence: The record for Staff 3 contained evidence that Staff 3 had resided outside of the state of Virginia within the preceding five years. The record for Staff 3 contained a documented hire date of 10/11/2023. There was not a child abuse and neglect registry check from all of the states in which the individual had resided in the preceding five years available for review during the inspection. Staff 3 was observed working during the inspection.
Evidence: The record for Child 1contained a documented start date of 10/30/2023. The most recent immunization record in the record for Child 1 was dated 10/03/2023; additional immunizations have not been obtained every 6 months from 10/03/2023 until the time that the child turned 2 years old.
Evidence: There were 20 children observed in the toddler group at approximately 10:00 am participating in free choice activities. The maximum group size for this age, based on an allowable variance for ratio issued to the center, is 18 children.
Evidence: There were 20 children observed in toddler class. There were 3 staff observed supervising the group of children. Staff 1 identified Child 2 as the youngest child in the group. Child 2 was determined to be 16 months old. The required staff-to-child ratio, based on an allowable variance for ratio issued to the center, for children 16 months up to 24 months is 1:6. A total of 4 staff was required for this group of children.
Based on observation, the center failed to ensure that the diapering surface shall be cleaned with soap and water and sanitized after each use.
Evidence: Staff 3 was observed changing a diaper. Staff 3 did not clean or sanitize the diapering surface after changing a diaper.
Evidence:
1. There were 5 children observed in the Infant Room; there were 6 children marked present on the attendance record. The attendance record did not document the departure of each child as it occurred.
2. There were 20 children observed in the toddler group; there were 17 children marked present on the attendance record. The attendance record did not document the arrival of each child as it occurred.
3. There were 19 children observed in the school-age group; there were 15 children marked present on the attendance record. The attendance record did not document the arrival of each child as it occurred.
