Inspector Notes

An unannounced, onsite monitoring inspection was initiated and completed on 12/05/2024. The on-site inspection began at 10:40 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were 22 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Staff 5 was the only staff member supervising the pre-k class. Staff 5 left the group of children to use the restroom. The children were not within actual sight and sound supervision of staff during the time Staff 5 was in the restroom.
Plan of Correction: Will discuss with staff and retrain the staff.
