The center is required to request background checks from any state in which the individual has resided in the preceding five years prior to employment. Staff #3, did not have documentation requesting criminal record check or a sex offender check from a different state prior to hire as it was conducted 3-weeks after hire.
Plan of Correction: Assigned staff members will conduct a full background check prior to hire date for all staff.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment and have been completed within the last 30 days of the date of employment.
