Inspector Notes

An unannounced monitoring inspection was conducted on 9/14/22 from 10:35am to 12:40pm with the Head of the Lower School and her Assistant. There were 12 four-year-old children in care, supervised by two staff. The children were observed eating a snack from home, washing hands, and reading books. Five child records and two staff records were reviewed. The Center has multiple staff with current certification in CPR and First Aid, as well as multiple staff trained in Daily Health Observation Training. There was no medication for children on site but the center has staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 9/23/21 and the most recent Health Inspection on file was dated 9/27/21. Standard 8VAC20-780-160-A
Based on review of two staff records and interview, the center did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Evidence: The record for Staff A, with a start date of 8/19/22, did not contain documentation of a negative TB test or screening
Plan of Correction: We have requested that the staff member obtain a TB test or screening right away. Moving forward we will be sure to obtain a negative TB prior to employment.

Based on review of two staff records and interview, the center did not ensure that the record for each staff contained documentation to demonstrate that the individual possesses the education required by the job position. Evidence: The record for Staff A, who is in the position of Program Leader for the program, did not contain documentation to demonstrate that the staff member has the education to qualify for the position.
Plan of Correction: We have requested that the staff member provide transcripts as soon as possible. Moving forward we will request staff provide documentation of education at hire.
